Champion Health Plan, USA (formally known as Renal Payer Solutions) is a Medicare Advantage Prescription Drug (MAPD) Chronic Special Needs Plan (C-SNP) and Renal delivery network for end stage renal disease (ESRD). We contract directly with the Center for Medicare & Medicaid Services (CMS) and as a value-based network with health plans, self-funded groups, and full-risk IPAs and medical groups.
Connecting C-SNPs to Improved Outcomes
In a 2020 study published in PubMed, researchers found a positive correlation between C-SNP enrollment and lower rates of hospitalization and mortality for patients with end-stage kidney disease. The findings suggest that the additional services and benefits provided by C-SNPs may improve outcomes, compared with standard care for this population.

Consistent treatments: Improved support programs enable more ESRD members to receive dialysis services successfully at home.
Hospital avoidance: Our Home Assist team provides home emergency care and 24/7 availability to Nephologist support care.

case management helps ensure in-home member support.
Telehealth services: Members have access
to telehealth services for all medical subspecialities, behavioral health, nutrition, social work and primary care with home nursing support when necessary. When appropriate, the Plan will also initiate remote patient monitoring and peer-to-peer consultations.

AI analytics and data reporting drives more holistic data.
Improved support for patients with poly pharmacy: Specialty pharmacy helps members take their medications as prescribed by their doctors, eliminating duplicate therapies and drug interactions.
